I understand that this application will be given due consideration, but its receipt does not imply that I will be employed by LBMC Strategic Staffing, LLC or placed with one of their clients. I certify that the information contained in this application is true and complete. I understand that my misstatement, omission, falsification, or misrepresentation of fact in this application may disqualify me from consideration for employment or, if I am hired, may result in disciplinary action up to and including discharge from employment. I also authorize persons, schools and organizations named to provide LBMC Strategic Staffing, LLC with relevant information that may be required. I further release all parties from any and all liability or claims for damages whatsoever that may result from this information’s release, disclosure or use. I authorize LBMC Strategic Staffing, LLC to conduct any credit or background investigations deemed necessary.

I understand that if I am accepted for a consulting assignment, I will be working for you on your payroll, at your clients’ premises. If I receive an offer of permanent employment from an LBMC Strategic Staffing client company as a result of your introduction, I will notify LBMC Strategic Staffing immediately. I understand that any information I learn while working for a client is to be kept confidential. While employed by LBMC Strategic Staffing on a consulting assignment, I will never render an opinion or sign a financial statement or tax return on behalf of LBMC Strategic Staffing, LLC.

LBMC Strategic Staffing has a very strict “NO DRUG POLICY”, and by signing this form I am consenting to submission to drug testing as required. I understand that my failure to comply with this policy will be grounds for immediate termination.

I agree, if employed by you, that if I ever make claims against you for personal injuries, upon your request I shall submit to examinations by physicians of your selection. I will hold you harmless from any claims including, but not limited to, personal injury or illness as a result of my providing false or misleading information on this application. I agree to take a drug and alcohol test if I have an on-the-job injury while your employee.

I understand that if employed, my employment will not be for any fixed period of time and may be terminated by LBMC Strategic Staffing, LLC at any time, with or without cause.
